Why Semaglutide Slows Your Gut — and What That Feels Like

Semaglutide works by mimicking GLP-1, a hormone your body already makes after meals. Part of what it does is tell the stomach to empty more slowly and the intestines to move content along at a gentler pace. That is the mechanism behind the appetite control people pay for. It is also the reason so many users describe semaglutide bloating and semaglutide gas within the first few weeks of a dose increase.

The pattern is fairly predictable. Symptoms cluster in the two to four days after a weekly injection or after a step up in dose. People report a stretched, full feeling after small meals, belching, and gas that seems out of proportion to what they ate. Most of it settles as the body adjusts, but „most“ is doing a lot of work in that sentence. A 2023 review of GLP-1 gastrointestinal effects found that nausea, constipation and abdominal distension are among the most commonly reported adverse events, and constipation in particular can turn bloating into a persistent, uncomfortable problem.

Practical fixes that clinicians in the U.S. actually suggest: eat smaller portions more often, stop eating at the first sign of fullness rather than the last, cut back on carbonated drinks and sugar alcohols like sorbitol and xylitol, and treat constipation early with fiber and fluids rather than waiting a week. If gas is severe and comes with vomiting or pain that does not pass, that is a call to your prescriber, not a wait-and-see situation.

Diverticulitis, Ozempic, and the Questions Patients Ask

Ozempic and diverticulitis is a pairing that shows up often in clinic conversations, and the honest answer is that the evidence is still thin. Diverticular disease is extremely common in the U.S. — roughly half of adults over 60 have diverticula, and many do not know it. So plenty of people starting a GLP-1 drug already have the condition, which makes association easy to mistake for causation.

What does concern clinicians is the constipation piece. Hard, infrequent stools raise pressure inside the colon, and that pressure is one of the theorized contributors to diverticulitis flares. If you have a history of diverticulitis, tell your prescriber before starting semaglutide. Ask specifically about a bowel regimen — typically adequate water, a fiber supplement such as psyllium, and sometimes a stool softener — and about which warning signs should send you to urgent care. Fever with left-sided lower abdominal pain is the classic picture and needs evaluation the same day.

When to hold the next dose

Dehydration from vomiting or diarrhea is a real risk on these drugs, and it matters more if you take other medications. If you cannot keep fluids down for a day, or you have a flare of diverticulitis with fever, most prescribers will tell you to hold the injection until you are eating and drinking normally again. Do not restart at the same dose after a long gap without checking; the gastrointestinal side effects are worst when the drug level jumps.

How Long It Stays in Your Body — and Whether You Can Skip a Week

The half-life of semaglutide is about one week, which is why it is dosed weekly. That long tail is also why side effects can linger for several days after a missed injection and why a dose change takes a few weeks to fully express itself. Roughly four to five half-lives — about a month — are needed to clear most of the drug from your system, though individual variation is wide.

Can you skip a week of semaglutide? Usually yes, and patients do it for travel, surgery, illness or cost. The trade-offs are mild hunger returning and, more importantly, the risk of restarting too high. The manufacturer’s labeling allows a missed dose to be taken within five days; beyond that, you wait for the next scheduled day. If you have missed two or more weeks, ask your prescriber whether to step back down a dose level. Many will say yes, and that single decision prevents a miserable week of nausea.

Anyone searching for a semaglutide weight loss program in Maryland or elsewhere should know the practical landscape: prescriptions require a licensed prescriber, compounded versions have been restricted as brand supply has recovered, and insurance coverage varies enormously. Maryland’s Medicaid program covers some GLP-1 drugs for diabetes but has tighter rules for weight loss, and commercial plans often require prior authorization with a documented BMI and a trial of a lifestyle program. Call your plan before you assume the price.

Reading the Signals: Side Effects vs. Something Else

Not every stomach symptom on these drugs comes from the drug. A gallstone attack can look like severe upper abdominal pain radiating to the back, and rapid weight loss raises gallstone risk. Pancreatitis is rare but serious, presenting as steady pain boring into the back with vomiting. New or worsening heartburn deserves a look too, especially in people over 50.

A simple rule helps sort it out. Bloating and gas that improve with smaller meals, less carbonation and better bowel habits are usually medication-related and manageable. Pain that wakes you at night, fever, vomiting that will not stop, or a change in bowel habits lasting more than a couple of weeks are not, and they deserve a workup. Keep a short log of what you ate, your bowel movements, and the days you inject. That record makes the next visit far more productive, and it helps your prescriber tell the difference between an expected adjustment and a problem worth chasing down.
